Children’s Case Management Services

Case management connecting children with serious emotional disturbance to services and supports.

Children's Case Management (sometimes called Youth Case Management or Care Management) is an ongoing service that a child may receive after being referred through SPOA and found eligible. Case managers work directly with the child and family to:

Assess strengths and needs

Develop goals and service plans

Coordinate care among providers, schools, and community agencies

Connect families to resources and supports

Monitor progress over time

Children’s Case Management Services provided by MHA in Ulster County, Astor Services, Family of Woodstock, and Northern Rivers assess strengths, develop service plans, and link children with serious emotional disturbance to treatment, housing, and educational supports; applications are submitted through the Ulster County SPOA.
